Aboriginal Health Practitioners
GRAMS runs an Aboriginal Health Practitioner First Program which is integral to our holistic health service delivery to our community.
AHPs are registered professionals who bring together clinical skills and cultural knowledge to deliver holistic, person-centred care. They work closely with individuals, families, and communities to ensure health services are accessible, respectful, and culturally safe.
What Do AHPs Do?
Our Aboriginal Health Practitioners:
- Conduct health assessments, screenings, and chronic disease management
- Provide health education and promotion tailored to community needs
- Support patients in understanding and navigating the health system
- Advocate for culturally respectful care within multidisciplinary teams
- Collaborate with local services to ensure continuity of care
