Kundimia Community Home Support Program  (CHSP) services are available to Aboriginal and Torres Strait Islander Elders or people living with disability to help live independently at home. 

Centre Based Day Care

Centre-based day care (CBDC) consists of activities conducted at the centre. A broad range of activities are provided to cater to client’s needs and interests.

Activities can include some group excursions, weather permitting.

Domestic Assistance

HACC staff support a client in their own home with identified everyday household tasks that they are unable to manage without assistance.
HACC staff can also assist clients with bill-paying, banking and shopping.

Home Maintenance

HACC home maintenance refers to assistance with minor home repairs and maintenance to a client’s home, garden or yard to maintain safety, independence and access for the client within the home.

Meals on Wheels

HACC staff deliver pre-prepared meals to clients in their home Monday to Friday including public holidays.

Personal Care

HACC can provide personal care to support clients with daily self-care activities such as showering and dressing. The primary focus of this support is on enabling client’s to regain and/or retain their independence in self-care. HACC staff can also support clients with medication administration and prompting and putting on Ted stockings.

Transport

Transport services support clients to remain living independently in the community by providing access to community activities and services, including social activities, shopping and medical appointments.
