As an Aboriginal Health Practitioner, you will play a vital role in delivering culturally safe primary healthcare, health education, and community support while building strong relationships within the community.

About the Role

Reporting to the Clinical Operations Manager, you will:

  • Deliver primary healthcare services in the clinic and community.
  • Conduct health checks, client follow-up, and health assessments.
  • Provide health education and health promotion activities.
  • Support clients to access appropriate healthcare services and referrals.
  • Collaborate with GPs, nurses, specialists, and other service providers.
  • Contribute to quality improvement initiatives and positive health outcomes.

About You

To be successful, you will have:

  • Current registration as an Aboriginal Health Practitioner with AHPRA.
  • Strong understanding of Aboriginal health issues and culturally safe practice.
  • Excellent communication and relationship-building skills.
  • Experience working with Aboriginal communities and multidisciplinary teams.
  • Knowledge of health promotion and community engagement.
  • Experience using Communicare or a similar clinical database system (desirable).
  • Current Driver's Licence, National Police Clearance, and Working with Children Check (or ability to obtain).
