Health Navigator
Location: Geraldton
The Health Navigator provides a single, consistent point of contact for children aged 5–9 years and their families referred through the On Track to Thrive (OTTT) program. The role supports families to access, coordinate and remain engaged in culturally safe health care across GRAMS services and external providers, including tertiary and metropolitan services, while reducing fragmentation, duplication and missed follow-up.
JOB SPECIFIC COMPETENCIES
General skills, experience and knowledge
ESSENTIAL:
- Registration as a Registered Nurse (RN) or Enrolled Nurse (EN) with AHPRA.
- Demonstrated experience in child health, primary health care, community health or care coordination.
- Strong understanding of trauma-informed practice.
- Demonstrated commitment to culturally safe practice with Aboriginal children and families.
- Experience coordinating care across multiple services and systems.
- High-level communication and relationship-building skills.
- Strong Organisational, documentation and follow-up skills.
- Ability to work flexibly across clinic, community and outreach settings.
DESIRABLE
- Experience working within an Aboriginal Community Controlled Health Organisation (ACCHO).
- Knowledge of Communicare or similar Patient Information Management Systems.
- Experience working with pediatric, mental health or SEWB services.
- Experience supporting families to access tertiary or metropolitan health services.
RESOURCES AND SUPPORT PROVIDED
- Laptop, mobile phone and access to GRAMS vehicle for work-related travel
- Structured clinical and reflective supervision
- Support from AHPs, ALOs, SEWB staff and clinical teams
- Strong clinical governance and accreditation framework (RACGP, AGPAL)
PRACTICE REQUIREMENTS
- You need to obtain a National Police Clearance.
- A current Working with Children Clearance or able to obtain one.
- A current unrestricted “C” class driver’s license.
- You can be tested for having used drugs at any time.
