The Geraldton Regional Aboriginal Medical Service (GRAMS) is seeking a passionate and dedicated Integrated Team Care Coordinator to join our team. This role is designed to support Aboriginal and Torres Strait Islander individuals with chronic conditions, ensuring they access the appropriate services to manage their health and address other barriers to care.
Key Responsibilities:
- Coordinate and support clients in accessing specialist and allied health services for chronic disease management.
- Develop strong working relationships with other healthcare professionals and services to ensure holistic patient care.
- Provide support for GP care management plans and assist with chronic disease management reviews.
- Work with Aboriginal clients to develop self-management skills for chronic conditions.
- Coordinate and arrange services for clients, ensuring they attend appointments on time and participate in regular health checks.
Essential Qualifications and Experience:
- Relevant clinical qualification (e.g., Aboriginal Health Worker/Practitioner)
- Current registration with AHPRA.
- Experience in a similar role within the health or community sector.
- Knowledge of Aboriginal culture and health issues.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
Desirable:
- Experience working with the Indigenous community and in rural health settings.
- Familiarity with the use of Communicare.
