Setting up home visits with Doctors, Registered Nurse and Health Worker.
Liaison with the Geraldton Regional Hospital.
I am responsible for the delivery of services to the Murchison region, this includes the GRAMS Mt Magnet clinic.
The main objective is to increase access to health services to Aboriginal people. I also coordinated visiting multidisciplinary teams, as part of the Rural West Health Medical Outreach Indigenous Chronic disease and rural Outreach funding programs.
A nurse practitioner is a registered nurse
with the expertise to diagnose and treat
people of all ages with a variety of acute
or chronic health conditions.
Nurse practitioners work directly with you and provide a comprehensive services including:
• assessment and diagnosis of your health problems
• referral for necessary x-rays and blood tests
• minor procedures
• prescription of medications
• referrals to other health practitioners
• education, counselling and support
Main role : after hours clinic: Mon –Friday 6-9pm
& chronic disease 1pm -6pm
